Please use this form to request Academic Supportive Measure(s) from the Title IX Office.

IMPORTANT NOTICE: Please be aware that requests for academic supportive measures filed late in the semester, when they could have been filed earlier, may lead to a denial of the request. Timely submission of requests ensures that we can provide the necessary support and accommodations effectively.

Examples of Timely Submission: 1. Submitting a request for exam accommodations at the beginning of the semester or as soon as the need arises. 2. Requesting extensions or modifications to assignment deadlines promptly after experiencing a significant event or change in circumstances. 3. Informing the Title IX office of any issues impacting your academic performance as soon as they occur, rather than waiting until the end of the semester. Consequences of Late Requests: 1. Late requests may not be accommodated due to insufficient time to implement the necessary measures. 2. Delayed submissions can result in denial of the request, impacting your academic performance and progress. 3. Filing requests late in the semester may limit the options and resources available to support you effectively.
